At the core of this innovative approach lies a protein called CRMP2, a crucial regulator of the Cav2.2 calcium channel. Researchers previously discovered a peptide known as the calcium channel-binding domain 3 (CBD3), derived from CRMP2. When introduced into cells, CBD3 acted as a decoy, preventing CRMP2 from binding to the calcium channel's inner region, resulting in reduced calcium influx and decreased neurotransmitter release, ultimately leading to reduced pain in animal studies.
Recognizing the challenges of using peptides as drugs due to their short duration of action and susceptibility to stomach degradation, scientists sought to develop a small molecule drug based on CBD3. Focusing on the 15 amino acids composing CBD3, they pinpointed two amino acids responsible for inhibiting calcium influx and mitigating pain, laying the foundation for designing a small molecule drug.
In collaboration with the University of Pittsburgh, researchers employed a computer simulation to screen 27 million compounds, searching for a small molecule that could mimic the action of CBD3's amino acids. This rigorous process identified 77 potential compounds, which were subsequently tested experimentally. This narrowed the selection down to nine compounds, evaluated using electrophysiology to measure reductions in electrical currents through calcium channels. The standout performer among these compounds, named CBD3063, disrupted the interaction between the CaV2.2 calcium channel and the CRMP2 protein, diminishing calcium entry into the channel and reducing neurotransmitter release.
Remarkably, CBD3063 proved effective in alleviating pain in mouse models for various pain types, including injury-induced pain. In a head-to-head comparison with gabapentin, CBD3063 required significantly lower dosages (1 to 10 mg vs. 30 mg) to achieve pain reduction. Collaborations with researchers at Virginia Commonwealth University, Michigan State University, and Rutgers University further demonstrated CBD3063's efficacy in treating chemotherapy-induced neuropathy, inflammatory pain, and trigeminal nerve pain—all without the side effects associated with gabapentin use, such as sedation, cognitive changes, or alterations in heart rate and breathing.
This groundbreaking research paves the way for a new era in pain management, offering hope to millions of individuals suffering from chronic pain by introducing a small molecule that indirectly targets calcium channels, outperforming traditional medications while minimizing side effects. As researchers continue to explore this promising compound, the future of pain relief looks brighter than ever.
